Multidisciplinary Care Team
Our hospice care program is led by a multidisciplinary team of healthcare professionals dedicated to providing comprehensive care. This team typically includes:
- Physicians – To manage and monitor the resident’s medical needs.
- Nurses – Skilled in palliative care, nurses play a key role in managing symptoms, providing pain relief, and offering daily care.
- Social Workers – To offer counseling, emotional support, and assistance with any financial, legal, or practical concerns families may have.
- Chaplains or Spiritual Counselors – Available to provide spiritual care and guidance, regardless of the resident’s faith or beliefs.
- Assistance With ADLs – Clinical nurses and nurse aides assist with personal care, including bathing, dressing, and feeding, ensuring residents or guests are comfortable.
- Volunteers – Trained volunteers offer companionship and support to residents and their families, helping with daily tasks or simply providing a comforting presence.
Pain and Symptom Management

We use a combination of medications, therapies, and other approaches to control pain, nausea, shortness of breath, and other common symptoms of terminal illness.
